Professional Assessment
Our role is take on all the onerous tasks and responsibilities of arranging, and managing the care and services for seniors and their families. In doing so we provide our clients and their families the opportunity to do what matters most – making the most of the precious time they have left together.
How do we do this?

The assessment is the foundation of our work. So often the “apparent” problem or challenge is a symptom of something else that is not obvious. For example-we often hear from a son or daughter that their elderly parent is encountering cognitive problems. After the assessment we find that there are multiple factors involved (e.g. of poor nutrition, depression, complications with medications, poor sleep patterns, emotional upsets, lack of social interactions, etc…)

The plan flows from the assessment. We make recommendations that will enhance the safety, health, and lifestyle of our clients. We can assist in the coordination of any of the care and services required. Ongoing monitoring is essential to determine what progress is being made and if any changes are required.
The SCSC team has a “community of care” that support our clients. We work independently with the agency, provider, or facility that will best serve the needs of our clients. With ongoing monitoring, we will immediately recommend or make any adjustments or changes that are necessary.
