The Senior Care and Services Team
The SCSC team is dedicated to assisting our clients in gaining what they value most:
- Independence
- To continue live in their home own home ( or for some to move to an environment that will enhance their independence and freedom)
- To pass on a legacy to their heirs
- Privacy
- Not be a burden to their children
The SCSC team has a “community of care” that support our clients. We work independently with the agency, provider, or facility that will best serve the needs of our clients. With ongoing monitoring, we will immediately recommend or make any adjustments or changes that are necessary.
The SCSC Team has many years of experience in the field of care and services to the aging.
Our “in house staff offers the following:
- Assessments
- Developing and implementing the Care and Service Plan
- Ongoing monitoring of client progress
- Accompany to Physician visits
- Ensure insurance is being utilized to the fullest extent possible
- Nutritional Health Counseling
- Emergency support
- Referral to, and coordination, with agencies, facilities and providers
- Information about assistive technology, and coordinate installation
- Counseling for family members and clients as well as client /family counseling.
- Assist in transition and placement in new situation (senior communities, apartments, condos, assisted living, Nursing Homes, Alzheimer/Memory Impaired facilities, or relatives home.
- For everything else we refer as needed to the professionals in our “community of care”.
As a result of many varied contacts with seniors in his boyhood, the owner Nick Meima had a vision of working with the elderly as a career when he was first went to College. He was one of the first eighteen people in the United States to earn his Masters Degree in Gerontology.
At age 24 Nick began as the Executive Director of large continuing care retirement communities ( with a range of independent, assisted, short term rehabilitation, skilled care, long term care, and hospice care. Over a period of 16 years he was Director of several communities. Prior to his work in Geriatric Case Management , he and his partner-created and operated a successful Home Health company, which continues to thrive today. Nick also consults with several senior facilities regarding marketing and operational issues.
