The primary needs of our clients revolve around meeting the day-to-day challenges. Once the plan is in place, we work closely with our clients and the care giving team to ensure that the services are meeting the needs of your loved one, and to see what the progress has been made towards more independence.

The SCSC team is dedicated to assisting our clients in gaining what they value most:

  • Independence
  • Privacy
  • To continue live in their home own home (or for some to move to an environment that will enhance their independence and freedom)
  • Not be a burden to their children
  • To pass on a legacy to their heirs

Interactions with physicians, proper nutrition, exercise, social engagement, and personal hygiene are essential for achieving maximum independence. Our nurses can accompany our clients to medical appointments. We can help with transportation, scheduling and coordinating appointments. This is a one of the most important services we offer. Clients will frequently be anxious about appointments or procedures, and tests. As a result client do not accurately report about their condition and consequently the professional cannot make the correct diagnosis or recommend the proper course of treatment. When adjustments in medication are needed, and the nurse can help sort out which medications may be most helpful. Finally the nurse can be the “translator” of what the physician or practitioner said, and can report to the family and care giving team.

Advocacy on behalf of our clients is one of the important services we provide. Whether it’s a physician, issues with Medicare or insurance carrier, or any other professional, we ensure that our clients are receiving the best service and care possible.

Hospitalization will at times be necessary for some of our clients. Our goal after discharge is to help our clients with all the care and support they need to regain maximum health and vitality. In addition to this, we do everything possible to avoid a readmission to the hospital. Unfortunately, there are often complications associated with stays in hospitals and care facilities and readmissions can severely tax the clients overall health and well being.

Nutrition is one of the most common issues with our elderly clients. “ Food just doesn’t taste good” or “It’s too much work to cook” or “I don’t feel like shopping.” These are statements we hear from many of our clients. We have a nutritional health counselor on our team that will assist in developing a plan that will meet taste preferences and essential dietary requirements to help our clients regain vitality and strength.

Medication management is a common issue with the elderly. We will ensure that medications are set up properly, taken as prescribed, and then monitor to see if there are indications of a negative response to the medications. If so we will communicate the problem to the physician to immediately resolve the problem.

Fall prevention is critical to remaining healthy and independent. There are many complications that may arise in the event of a fall which often have long term debilitating consequences. We will provide a plan and ongoing monitoring to make sure that the risk of a fall is less likely.

Counseling with clients and caregivers is sometimes needed. We have years of experience in dealing with client issues, and issues relating to their families. We can help sort our some “knotty” underlying issues that sometimes surface as circumstance force them to the surface.

We have developed a “community of care”, of the best providers in their respective areas of expertise. However, we work independently from any agency, provider, or facility. This gives us the flexibility we need to be able to implement changes as needed, so that we can best serve the needs of our clients.

Our “In house staff” provides:

  • Assessments
  • Care and Service Plan
  • Ongoing monitoring of client progress
  • Accompany to Physician visits
  • Ensure insurance is being utilized to the fullest extent possible
  • Nutritional Health Counseling
  • Emergency support
  • Referral to, and coordination, with agencies and providers
  • Information and arranging installation of assistive technology
  • Counseling for family members and clients as well as client /family counseling.
  • Assist in transition and placement in new situation (senior communities, apartments, condos, assisted living, Nursing Homes, Alzheimer/Memory facilities, or relatives home.
